Minor Consent for Treatment by University Health Services (UHS) Providers

UHS is dedicated to providing high-quality medical care to all UT Austin students. In accordance with Texas state law, any currently enrolled UT student who is under the age of 18 years old must obtain or meet the qualifications below to be treated by a University Health Services provider.

Please be aware that accessing healthcare services in the community can be challenging for minors. UHS strongly advises UT Austin student minors to complete the minor consent for treatment process with UHS to simplify healthcare access while on campus.

Option #1

Obtain parental consent.

Option #2

Meet any of the qualifications for self-consent, which are listed below:

  • Currently be in active duty with the armed services of the United States of America.
  • Reside separately from your parent(s)/guardian(s) and manage your own finances regardless of the source of your income.
  • Consent to the diagnosis and treatment of an infectious, contagious, or communicable disease reported by the Texas Department of Health.
  • Be unmarried and pregnant and consent to medical treatment, other than abortion, related to the pregnancy.
  • Consent to examination and treatment for drug or chemical addiction, drug or chemical dependency, or any other condition directly related to drug or chemical use.
  • Consent to counseling for suicide prevention, chemical addiction or dependency or sexual, physical, or emotional abuse.

If you meet the criteria for self-consent (option #2), call 512-471-4955 to schedule your appointment. The scheduler will send an electronic minor consent form to you through the MyUHS portal for you to sign during the call.

If you do not meet the criteria for self-consent (option #2), your parent/guardian must complete the Parent Consent for Treatment of a Minor form.

  • Upon completion of the form, please go to healthyhorns.utexas.edu and click on the MyUHS tab in the main navigation.
  • Click on Messages, then click new message. Select the option: I want to submit my Parental Consent for Treatment of a Minor. Upload your signed consent as an attachment and hit send.
  • Once the documentation is uploaded and approved by our staff, you will receive a secure message within your MyUHS portal notifying you that you are approved to schedule medical appointments at UHS.

If you are under the age of 18 years old upon enrollment at UT Austin, cannot obtain or meet the criteria listed above and need medical care from a UHS clinician, please call 512-471-4955.
