Advance directive is a set of written instructions specifying what actions you would want taken for your health if you should become unable to make decisions due to illness or other incapacitating factors. You have the right to provide an advance directive to University Health Services (UHS) and have UHS providers and staff comply with your instructions.
For assistance with developing an advance directive, contact Legal Services for Students in 3.410H in the Student Services Building (SSB) or call them at (512) 471-7796.
When you have a completed advance directive, take it to the UHS Appointments desk or any UHS check-in desk. The staff at these areas will accept the advance directives document and route it to be incorporated into your health record.
